The Illinois State Fire Marshal moved office locations. They are returning any Applications for Certificate of Operations and Inspection Forms that are sent to the old address. I wanted to let you know to avoid wasted time and effort on your part as well as delays to getting your elevator certificate which could result in a late fee. Here is a link to the current forms:
