Today I went to quote a job and ended up fixing the customers power loss issue. Coincidentally as I arrived they lost power. What I found shown in the images below was probably caused by a loose connection at the bottom terminals of the safety switch and circuit breakers, a bit of load on the circuits which would have caused an arcing. Safe to say the connections are tight and there shouldn't be any arcing.
