In this incident, a gas-fired condensing boiler was installed as the primary heating source in a large residential home, despite being undersized for the property. The installation was completed without the required permit. An investigation found multiple deficiencies, including improperly sized venting, non-compliant vent terminations with insufficient separation between the air intake and exhaust, and incorrect air-fuel adjustment at low fire. Over a two-year period, six occupants were exposed to carbon monoxide.
