On the Tuesday May 8th 2018, exactly a week after the yearly May day celebrations, a holiday to celebrate and address concerns of workers throughout the world, a horrid story was reported in the Daily Graphic of an occupational injury sustained by a factory worker, who lost his wrist in the conduct of an equipment maintenance activity. Though the story focused on the good work, intricate and painstaking surgery conducted by a team of doctors led by Dr Edem Kofi Anyigba at the Korle- Bu Teaching Hospital to re-attach the severed wrist, I perceive the accident from a different perspective and seek to delve deeper into the failures that could have contributed to the occurrence of such an accident.
