On the Tuesday May 8th 2018, exactly a week after the yearly May day celebrations, a holiday to celebrate and address concerns of workers throughout the world, a horrid story was reported in the Daily Graphic of an occupational injury sustained by a factory worker, who lost his wrist in the conduct of an equipment maintenance activity. Though the story focused on the good work, intricate and painstaking surgery conducted by a team of doctors led by Dr Edem Kofi Anyigba at the Korle- Bu Teaching Hospital to re-attach the severed wrist, I perceive the accident from a different perspective and seek to delve deeper into the failures that could have contributed to the occurrence of such an accident.

Do you know that the massive gas explosion at Atomic Junction in Accra claimed seven lives, and left 132 others badly injured, and this was the eighth gas explosion since 2014?
