Basic reasons of battery failure
1-battery was inactive for long time
2-battery was discharged below discharge level which is 1.7v * number of cells ex. 48v battery 1.7*24 = 40.8v
3- continue using battery while there is bad cell untreated
4- battery was exposured high / low temperature
5- charging process was interrupted frequently
6- using improber charger
7- no maintenance or periodic check is made for the battery

Battery sulfation
It is anormal process take place during normal operation of battery , as known thecurrent flow through the battery is aresult of chemical reaction inside the cells through the electrolyte
the chemical reaction breaks down the sulphuric acid , as the break down starts free sulfur jobs begin to make crystals bind or stick to the lead grid plates in the form of lead sulfate crystals as the process of battery operation is repeated the crystals grow and harden coating the lead plates of the cell and affect the efficiency of the whole Battery , finally sulfation is due to normal operation of battery and is not avoidable but can be treated by battery regenration if there is no open or short cell
