Requirements for MOBILE PASSPORT SERVICES
NEW APPLICANT
• PSA/NSO Birth Certificate
• Baptismal Certificate (if Birth is delayed Registration)
• PSA/NSO Marriage Certificate for married woman
• Two (2) digitized government issued ID not including Philhealth, TIN, PAGIBIG
• Two (2) supporting documents
RENEWAL
• If Electronic Passport (EA/EB/EC)
o Original Passport and photocopy of 1st page and last page
o Two (2) valid IDs
o PSA/NSO Birth Certificate (below 18 years old)
o Baptismal Certificate (if Birth is delayed Registration)
• If Machine Readable or Handwritten Passport
o Original Passport and photocopy of 1st page and last page
o PSA/NSO Birth Certificate
o Baptismal Certificate (if Birth is delayed Registration)
o PSA/NSO Marriage Certificate for married woman
o Two (2) digitized government issued ID not including Philhealth, TIN, PAGIBIG
o One (1) supporting document
MINOR APPLICANT (below 18 years old)
• Original Passport and photocopy of 1st page and last page of parent or applying with parent
• PSA/NSO Birth Certificate
• Baptismal Certificate (if Birth is delayed Registration)
• Valid ID (8 years old and above)
• Marriage Certificate of Parents if applying with father
IF MOTHER IS IN ABROAD
• Original Passport and photocopy of 1st page and last page of parent or applying with parent
• PSA/NSO Birth Certificate
• Baptismal Certificate (if Birth is delayed Registration)
• Valid ID (8 years old and above)
• Marriage Certificate of Parents if applying with father
• Affidavit of Support and Consent duly authenticated by the nearest Embassy
• Special Power of Attorney (SPA) duly authenticated by the nearest embassy
• DSWD Clearance
PLEASE NOTE
• If Birth/Marriage Certificate is blurred, secure a Certified True Copy or Form 1A from Local Civil Registry Office
• If Birth/Marriage Certificate contains mistake/discrepancy in details, kindly apply for petition of correction to correct the information
• If ID has discrepancy, please have it corrected immediately
• Please prepare the exact amount; Php 1,200 for the passport fee and Php 150 for the courier per person.
• No early release and no walk in applicants
• No cancellation of passport upon release
• Please wear modest clothes; no sando
• Please remove earrings, contact lens, necklace, headband, and other forms of facial jewelry
• Applicants with medical alteration must bring a medical certificate
• Bring Original and Photocopy of Documents

• Seaman’s Book
• Elementary or High School Form 137 or TOR with readable dry seal
• Government Service Record
• NBI Clearance
• Readable SSS-E1 Form or Microfilmed copy of SSS E-1 Form
• Voter’s Certification, List of Voter’s and Voter’s Registration Record (1 Set - please attach receipt)
• OMA Certificate (for muslim)
• Affidavit of Loss (for loss passport)
REQUIREMENTS FOR REPLACEMENT OF LOST PASSPORTS
GENERAL REQUIREMENTS:
• Personal appearance
• Confirmed appointment for applicants with expired passports and issued travel documents (lost-valid passport applicant may proceed directly to the Consular Records Division at DFA-Aseana without an appointment)
• Duly accomplished application form - may be downloaded from the DFA website, www.dfa.gov.ph
• Requirements for the first-time passport applicants are still prescribed

In case passport was lost and cannot be found:
Submit Notarized Affidavit of Loss (with detailed explanation on when, where and

• If the lost passport is still valid, submit Police Report.
• Photocopy of first page of lost passport (if available)
Please take note that there will be a 15-day clearing period prior to the processing of application for replacement of a lost valid passport.
In case passport was already declares lost by the applicant but eventually was found and returned to him/her within the 15-day clearing period:
• Submit a Notarized Affidavit of Retrieval (with detailed explanation on when, where and how passport was found)
• Original and photocopy of data page and last page of passport
