No telling what caused the arcing here, however, the screw on the device was not tightened properly and on one side the insulation was under the terminal. As a result, we found some smoke residue and power wasn’t flowing to the other device.

Yes, we do come across this type of situation. Some handymen, electricians or homeowners may install ceiling fans on boxes not rated for ceiling fans. If you have a ceiling fan that’s wobbling excessively or hanging from the ceiling, give me a call. I have a fix for that.
