This broken part is off of 4000 amp fused switch. Cost a customer $49,000. We determined either a manufacturing flaw in the part or faulty design that shared the pin. Usually, we can point most failures toward bad maintenance, but if this pin sheared during the opening process because of the impact of the spring and the mechanism slamming against each other then it would be bad design actually doing maintenance, potentially causing the failure.

Just say no. If it doesn’t look safe if probably isn’t.

Showed up on IR we performed early summer. Good catch by my team. Eventually would have been catastrophic.
What the hell was this guy thinking and how the hell was he performing this work.
We talk about the dragon. It refers to the potential catastrophic failure that can be just behind every cover we remove. You will never see the dragon coming just know it’s there and be prepared.
