This is what a complete device failure looks like. This was an outlet for an EV charger. If you have had your EV charger plugged in for awhile and not checked it, now might be a good time! Look for any signs of heat damage on the face of the outlet. In this particular case, the connections were not properly tightened and the outlet was not correct for the application. If your EV charger outlet does not indicate on the face of the outlet that it is for EV vehicle charging, it needs to be corrected.
Solution: call Joshua Tree Electric.
