"Flawed from day one," states a new report on the tragic Surfside condominium collapse. The original plans specified structural columns that were too narrow to accommodate enough rebar, meaning that contractors had to choose between cramming extra steel into a too-small column or inadequately attaching floor slabs to their supports. Thankfully building codes have come a long way in recent decades, requiring stronger steel reinforcement and concrete less susceptible to salt-water intrusion.

Engineers to the rescue. Over a year ago, the Golden Gate bridge was modified with wind barriers for pedestrians. However, it was never acoustically tested and the bridge now sings a mighty song during wind storms.

In a retrofit-gone-wrong, the iconic bridge was turned into the world's biggest music instrument. Now, engineers are trying to fix it back.
