A TRUE STORY.
61-year-old woman.
Not overweight.
Does yoga.
On no medications.
Sees her P*P regularly.
Looked healthy.
Her coronary calcium score — CAC — was ZERO.
Traditional reactive medicine said:
“No symptoms? No need to repeat testing for 5–10 years.”
But in less than 5 years, her CAC jumped to ~160.
By traditional standards, this was still considered low concern.

We ordered a CT coronary angiogram — CTA.
Pushback came immediately:
“Unnecessary.”
“Overdiagnosis.”
“Too aggressive.”
The CTA showed a 75% blockage in her LAD — the so-called “widowmaker” artery.
Yesterday, she underwent cardiac catheterization.
What the catheterization revealed:
• 90% blockage in the LAD
• 90% blockage in a major LAD branch
• TWO stents required
She never had classic chest pain.
This is the problem with reactive medicine:
• Wait for symptoms
• Wait for angina
• Wait for the heart attack
• By then, it may be too late
Heart disease in women is frequently silent.
Sometimes the FIRST symptom is sudden death.
