π Big News: Advocate Access NW is Officially Ready! π
We are beyond thrilled to announce that Advocate Access NW is now 100% ready to serve you! We know that change can be scary and your time is precious, so we've made our onboarding process as smooth and streamlined as possible!
For Clients:
Complete a Release of Information (ROI):
If you receive services through the CCMHI/DD Program, use their ROI.For ISN, use their ROI.
Both forms are available on our website, or you can ask your Service Coordinator or Personal Agent for assistance. We also have copies at our office: 2155 Gable Road.
The ROI allows us to receive your Individual Support Plan (ISP), Risk Identification Tool (RIT), Person-Centered Information (PCI), and other essential documents to create your support plan.
Receive Your New Client Packet:
Once we receive your documents from your Case Management Entity (typically within 48 hours), you will receive a New Client Packet via email. We've made the form incredibly user-friendly, thanks to your valuable feedback!
Onboard Your DSP:
If you already have a DSP, we will onboard them simultaneously so you can connect instantly once both processes are complete.
If you don't have a DSP, check out our DSP Connect! We'll help you meet available DSPs and find the perfect match for your needs.
Throughout this process, you'll have a dedicated Service Support Advocate to guide you every step of the way. You are NEVER alone in this journey. We can't wait to have you join our organization. We promise to provide services like no one has before!
