Roughly 60% of confined space deaths are the rescuers.

A worker goes down inside a vessel. He stops answering. The crew topside goes in after him, because that is what people do. The atmosphere that dropped the first worker has not changed in the ninety seconds since.

A harness that stops the fall is only half a system.

Section 141 of Alberta's OHS Code is specific about this. Fall protection training has to cover the procedures for rescuing a worker suspended in a fall arrest system. Not just how to wear the harness. How to get the person down.

Ask your crew today. If someone goes over the edge and the lanyard does exactly what it is supposed to do, who brings them down, with what gear, and how long does it take? If the answer is "we call 911," that is your gap.

Let go of the rope. Nothing moves.

That is progress capture, and it is the difference between a haul system and a hazard. This is a pre-established rope access haul and lowering rig — built on the ground before work starts, with a 3:1 mechanical advantage established to each descender so one unit of pull moves three units of load. The descent device takes the weight between pulls, which means a technician can stop, slide the rope grabs back up and re-establish the haul without losing an inch and without a second person tied up holding the system.

Coming back down: one hand on both rope tails, one hand on both descender handles, steady pace, load stays level. The whole operation runs from an accessible anchor at ground level.

Trained technique to IRATA and SPRAT standards. This is the rigging that sits on the Level 2 and Level 3 syllabus — the part of rope access that is not climbing.

Ask anyone on site what the fall protection height is in Alberta, and you will hear 3 metres. That answer is incomplete.

Alberta OHS Code section 139(1) sets four triggers for fall protection, and three of them can apply below 3 metres. The one missed most often: at a permanent work area, protection is required from more than 1.2 metres.

1.2 metres is a mezzanine edge or a loading dock, not a tower. It is exactly the kind of height nobody writes a hazard assessment for.

If your program is built around the number 3 and nothing else, it is built on one quarter of the section.
